2023-2024 CCEP Student Release Form

For legal purposes we changed the format of this form. There is now one form submission needed per student. THIS FORM IS TO BE FILLED OUT BY A PARENT FOR A STUDENT. PLEASE TYPE THE STUDENTS NAME IN THE FIELD THAT SAYS "YOUR NAME".

This Student Release Form is good for the school year. This form is valid through May 31, 2024. For every minor involved in the youth activities at the Christian Church of Estes Park, we require information and permission from a parent or guardian. This form MUST be fill out for EACH student in your household. When you fill out this form you are giving your permission that your child may participate in CCEP Youth activities and events during the 2023-2024 summer and school year.
